Formal follow-ups:  Initially there will be 2 formal follow-ups 1 every 3 months, costing $30 per formal follow-up.  Thereafter, the follow-ups become less, since conditions generally improve fairly quickly.  Depending on how chronic your condition is, it normally takes 1 yr for every 10 years of having a condition.   Chronic conditions are conditions that you have not had success with in curing, and are taking medications, either over the counter meds or medically prescribed medications.
